A package is good for 6 months. Unused visits may be refunded if you ask, less the individual visit price of the services you have already received.

No. Avicenna is a cash-pay practice and does not bill insurance. You may use an HSA, FSA or FLEX account.
We aren’t able to provide superbills.
No. Testing and supplements are priced separately. Testing is chosen for your situation, so its cost depends on which tests you need. We’ll tell you the cost before anything is ordered.
You have the right to a written Good Faith Estimate of your costs when you schedule care or whenever you ask. Prices are for patients located in Florida and Texas, and may change; the estimate you receive is the one that applies to you.
